A Declaration of Completion form (DOC) must
be completed for each student successfully completing an ICBC-Approved
Driver Education Course.
These numbered forms are distributed to Driver
Training Schools by ICBC's Driver Training Unit.
These forms record the successful completion of the course for the
student, the school and ICBC. Schools are required to maintain a
log of the disposition of these forms, including lost, stolen or
destroyed forms.
This form has two purposes:
- A student who completes an ICBC-Approved Driver
Education Course in their learner stage may be granted a 6 month reduction of their Novice stage if they remain at-fault crash-free and violation free during the first 18 months of the Novice stage
- The Ministry of Education may
allocate 2 credits towards a student's high school graduation
upon receiving proof that the student has successfully
completed an ICBC-Approved Driver Education Course.
A student will be required to provide a copy of this
form to their educational institution in order to apply
for these credits. The student must provide the
student copy (blue copy) provided by the driving school.
In the case where this copy is either misplaced or lost,
the Ministry of Education will accept a photocopy provided
by the driving school under the following provisions:
- must
be a clear, legible photocopy
- the photocopy
must be re-stamped to designate it as an original
- the driving
school should indicate on the form that it is a
'duplicate' or 'replacement' copy and must be initialed
by the school
A school who's course has been approved will be
sent with their approval package an initial supply of these forms.
