Purpose: Upon successful completion of a Driver Certification Course, including qualification on the required Assessment(s), the Driver Certification Facility must provide the trainee with a completed Driver Certification (MV2067) form or "Certification Form". The trainee must present this form at a Driver Licensing Office, Appointed Agent or Government Agent of ICBC when applying for an amended driver's licence.

	Certification Forms are distributed to Driver Certification Facilities by ICBC's Driver Training and Assessment Standards in Victoria
	Assessment Facilities must use the latest version of the Driver Certification (MV2067) form
	Driver Certification Facilities are required to ensure that no loss, theft or misuse of these forms occurs, and to immediately report any such occurrence to Driver Training & Assessment Standards
