Upon successful completion of a Driver Certification Course, including qualification on the required Assessment(s), a Driver Certification Facility must provide the trainee with a completed Driver Certification (MV2067) form or "Certification Form".

The trainee must present this form at a Driver Licensing Office, Appointed Agent or Government Agent of ICBC when applying for an amended driver's licence.

• Certification Forms are distributed to Driver Certification Facilities by ICBC's Driver Training Unit in Victoria
• Assessment Facilities must use the latest version of the Driver Certification (MV2067) form
• Driver Certification Facilities are required to ensure that no loss, theft or misuse of these forms occurs, and to immediately report any such occurrence to Driver Training & Assessment Standards
